What the self-pay prices actually are
The terms say it plainly: patients using the savings offer or NovoCare Pharmacy "pay $199 for each month of 0.25 mg & 0.5 mg. Offer for 2 monthly fills through December 31, 2026, then $349 per month for Wegovy 0.25 mg, 0.5 mg, 1 mg, 1.7 mg, or 2.4 mg and $399 per month for Wegovy HD 7.2 mg."
So the ladder is:
- **0.25 mg and 0.5 mg — $199**, for two fills, through December 31, 2026 - **0.25, 0.5, 1, 1.7 and 2.4 mg — $349** after that - **Wegovy HD 7.2 mg — $399** - **1.5 mg and 4 mg (the tablets) — $149**, with the 4 mg price "only available until August 31, 2026, then $199 per month"
0.25 mg and 0.5 mg are the titration doses you start on and move up from. So the $199 is doing two things at once: it is a real price, and by the terms attached to it — two fills, starting doses only — it is a price with a short life by design. The number that describes a year on Wegovy is $349.
Two expiry dates worth putting in your calendar
Both cheap prices here are time-boxed, and the dates are in the terms:
- The **4 mg tablet at $149** is available "only until August 31, 2026," then $199. - The **$199 injection offer** runs "through December 31, 2026," then $349.
A price with an expiry date is not the same as a price. If you are budgeting a year of treatment, budget the number that applies after the date, not before it.
"One month" is four pens
Novo defines a month as "1 box of 4 pens of Wegovy and 1 bottle of 30 tablets of Wegovy." Four weekly pens is a 28-day supply, not a calendar month — so roughly thirteen fills fit into a year rather than twelve. We hold every telehealth program we track to the same reading, because a seller's "/mo" is not automatically a calendar month.
If you do have insurance, price that first
The same page offers a very different deal to insured patients: "Pay as little as $25, subject to a maximum savings of $100/month." That cap matters — $25 is the floor, and the card contributes at most $100, so what you actually pay depends on what your plan leaves behind. There is also a Medicare route: "You may be able to pay $50 a month. Terms and conditions apply."
If a commercial plan covers Wegovy, that copay usually beats every cash price on this page. Check eligibility properly first — savings cards are narrower than the advertising suggests.
The compounded comparison, and what it is not
Across the 364 programs we track that publish a semaglutide price (counted August 2026), the median is about $180 a month and the lower quartile is $149. The cheapest program publishing a genuinely flat, month-to-month semaglutide rate — no plan term, no first-month framing — runs $93 a month. Set against $349 for maintenance-dose Wegovy, that is a wide gap. Two honest caveats, because the gap is not the whole story:
