Wegovy Pill Cost Without Insurance: $149 Start, Then What

Without insurance, the Wegovy pill costs $149 for the 1.5 mg starting dose, $199 for 4 mg, and $299 a month for the 9 mg and 25 mg doses. That's Novo Nordisk's self-pay price through NovoCare Pharmacy or its savings offer at a retail pharmacy12, and the TrumpRx coupon gives the same three prices3. The label moves you up one dose every 30 days, so $149 lasts one bottle and $199 lasts one more. From day 61 on, the pill costs $299 a month. The only way below that is Novo's 12-month subscription at $249 a month, sold through telehealth programs4.

The Wegovy pill price at every dose

The Wegovy label sets a fixed escalation: 1.5 mg for days 1–30, 4 mg for days 31–60, 9 mg for days 61–90, and 25 mg from day 91 on5. One "month" in Novo's price terms is one bottle of 30 tablets1. That lines up with the label's 30-day steps, so each step is one bottle.

Three things in that table matter more than the $149 headline:

  • The cheap price covers 30 days. You pay $149 for your first bottle only, because the label moves you to 4 mg after that.
  • Staying on a lower dose saves nothing after 4 mg. 9 mg and 25 mg are both $299. If you and your prescriber stop at 9 mg, the bill is the same as at 25 mg.
  • The 4 mg price has already gone up. Novo's March 2026 announcement priced both 1.5 mg and 4 mg at $1494. Its own terms then limited the 4 mg price to August 31, 2026. Today's NovoCare page and price guide list 4 mg at $19912. Don't trust a $149 4-mg quote dated before September.

TrumpRx shows a list price of $1,349.02 for a 30-tablet bottle of the starting dose3. The coupon is run for Novo by a discount-card company. It's for self-pay patients only. It isn't valid for a fill billed, even partly, to Medicare, VA, TRICARE, another federal or state program, or commercial insurance. If you have that coverage and use the coupon anyway, you agree not to claim the cost back or count it toward your deductible. Novo says it can change or cancel these offers at any time13.

What a year costs

Under the standard offer, a first year on the pill comes to about $3,338 for 12 bottles: $149 + $199 + ten bottles at $299. A daily tablet at 30 per bottle actually uses about 12.2 bottles in 365 days, so plan on roughly $3,400. Every year after the first is about $3,600.

The subscription: when $249 beats $299

On March 31, 2026, Novo launched multi-month self-pay subscriptions through telehealth partners4. The pill tiers are:

Novo says the subscription prices hold flat whatever your dose. That cuts both ways. In months one and two, $249 is more than the $149 and $199 you'd pay month to month. Over a first year it still comes out ahead: 12 × $249 = $2,988, against about $3,338 under the standard ladder. That's roughly $350 saved in year one and about $600 in a full maintenance year, which matches Novo's own "$600 a year" figure for the pill4.

The catch is the commitment. Novo's release says billing, cancellation and renewal terms "vary by platform." It names Ro, WeightWatchers and LifeMD as launch partners, with others to follow4. Before you sign up, ask whether the year is charged up front or monthly, and what it costs to leave early. A 3-month plan at $289 saves only $10 a month over the $299 standard price.

Pill vs pen: the per-month math

At maintenance, the pill costs $299 a month and the pen costs $349. The two "months" aren't the same length, though. A pill month is 30 tablets, or 30 days. A pen month is a box of four weekly pens, or 28 days1.

At list self-pay prices, the pill is about $900 a year cheaper than the 2.4 mg pen at maintenance. The pen's $199 intro narrows that only in the first two fills, and the intro ends on December 31, 20261. On the 12-month subscription the two forms cost the same.

The label adds one cost point. If you don't tolerate 25 mg, it says to consider switching to the 1.7 mg injection5, which is $349. So someone who can't stay on the top pill dose ends up paying the pen's price.

Daily-dose rules that change what you get for the money

You pay for a pill that only works if you take it correctly, and the label is strict about how5:

  • Take one tablet each morning on an empty stomach, with no more than 4 ounces of plain water. No other liquids.
  • Wait at least 30 minutes before you eat, drink or take other oral medicines.
  • Swallow it whole. Don't split, crush or chew it.
  • If you miss a dose, skip it and take the next one the following day.

If that routine doesn't fit your mornings, the weekly pen may be worth its higher price. The label also reports that the tablet raised levothyroxine exposure by 33% in a study, which matters if you take thyroid medication5. On results: in the OASIS 4 trial, oral semaglutide 25 mg produced about 14% average weight loss at 64 weeks, against about 2% on placebo7. That's close to what the 2.4 mg pen achieves. Novo reports 16.6% for people who kept taking the drug as directed6. The FDA approved the pill on December 22, 20256.

With insurance or Medicare

  • Commercial insurance: if your plan covers the pill, Novo's savings card lets you pay "as little as $25" a month, with a maximum saving of $100 a month. Government beneficiaries are excluded2. If your plan leaves you owing $250, you pay $150.
  • Medicare: from July 1, 2026 through December 31, 2027, the Medicare GLP-1 Bridge covers the Wegovy tablet at $50 a month, whatever your income8. To qualify you need Part D coverage, no GLP-1 already covered through your drug plan, no type 2 diabetes, moderate-to-severe sleep apnea or fatty liver disease, and a qualifying BMI and health condition. The $50 doesn't count toward your Part D deductible or out-of-pocket limit. Refills don't need a new approval if you stay on the same drug, even when the dose changes8.

Compounded oral semaglutide: cheaper, and not the same product

Of the 425 active programs we track, 138 offer an oral or sublingual GLP-1 route (counted October 2026). Of those, 132 publish a program price. The cheapest is $79, the lower quartile is $149 and the median is $199. Those numbers are each program's lowest published price, though, and for most programs that's the injection. Only 21 of the 138 publish a separate oral price. Examples from our October 5, 2026 re-reads:

  • Direct Meds: $249 a month for sublingual semaglutide, against $297 for its injection.
  • Luvo Health: $259 every four weeks for semaglutide tablets, so 13 charges a year. The first four weeks are half price.
  • MyDrHank: $249 a month for its tablet, month to month. The lower "from" price needs a 12-month prepay.
  • SkinnyRx: $149 for the first month of a semaglutide tablet. The price after month one isn't published.

So a compounded oral route usually runs $150–$260 a month. That's often less than the brand pill's $299, but the brand pill's $249 subscription overlaps the top of that range. Compounded oral and sublingual semaglutide are not FDA-approved and are not reviewed for safety, effectiveness or quality9. None of them is the tablet tested in OASIS 4. If you want the brand pill through a telehealth program, check whether the program bills a membership fee on top of the drug. Sesame, for example, lists the Wegovy pill at the same $149 and $299 drug prices, plus a care membership of $99 every 28 days, or $59 a month on a 12-month prepay.

Who the pill makes sense for

  • People who won't inject. It's the only FDA-approved semaglutide for weight loss without a needle.
  • Self-pay patients headed for maintenance. At $299 against $349, and 30-day against 28-day fills, it's the cheaper brand-name semaglutide at list price.
  • Anyone who can commit to a year. At $249 on the 12-month subscription, the pill and pen cost the same, so choose by routine, not price.
  • Medicare patients who qualify for the Bridge. $50 a month beats every cash price on this page.
